Neudove Health is an independent community referral partner offering psychiatric and mental health services for children, adolescents, and adults, with telehealth and in-person appointment options available.

Important Referral Notice
Referrals to Neudove Health are made by Kelechi Ezeadi, LCSW, following clinical sessions when psychiatric evaluation, medication management, or additional psychiatric support is clinically indicated.
This is not a self-referral pathway through Holy Trinity Wellness & Leadership Group. Clients and families should complete Neudove Health’s pre-registration process only after the referral has been discussed and recommended during treatment.

Insurance & Identification Documents
Neudove Health requests a copy of the front and back of the client’s health insurance card and a photo ID. These documents should be emailed directly to:

What Happens Next
After the pre-registration information is submitted, Neudove Health will use the information to create the client’s chart and verify insurance. Neudove Health will then contact the client or family regarding appointment scheduling and next steps.
